I-693 Medical for Change of Status form (complete these forms and bring them with you to the exam) The fee is $150.00.
PPD/Tb test $10.00 (will need come back 48-72 hrs. for interpretation)
RPR/Syphilis blood test $10.00
Immunizations are additional. We have Td (Tetanus) $55.00 and MMR $80.00. Click here for Table A or Click Here for required immunizations. The Zoster and HPV vaccines are no longer required.
