SAME-DAY PHYSICAL REQUIREMENTS
We do offer same-day examinations and Form I-693 certification only if you meet the following conditions.
- Immunization Records
You must bring with you all your records of immunizations as shown on TABLE A. If you do not have all of the required immunizations you may obtain them at your local County Health Department or from your personal physician. If you have had chicken pox in the past, or if you have had a blood test that shows immunity to Varicella, you do not need the Varicella vaccine. If you have proof by blood test for all three diseases (measles, mumps, and rubella) you do not need those vaccines. In order for your Td (tetanus) vaccine to be current, it must have been received in the last 10 years.
- PPD (Tuberculosis Skin Test)
Have your private physician, or County Health Department apply and read a PPD/Tb skin test. Make sure the report contains the date the test was applied, read and size in “mm”. If the test is 5 mm or more a chest x-ray is required. The x-ray report must be on the radiologist’s stationary and signed by the radiologist. If the x-ray is abnormal or you have any signs or symptoms of Tb you need to bring a note from the Health Department Tb Clinic that states you are disease free and not contagious.
- RPR/Syphilis Blood test
Have your doctor or the health county department perform a Syphilis blood test and bring the results with you.
If you bring all the listed information with you, and if Dr. Gordon certifies your examination, you will be able to take your completed forms with you.
